Service Coordination
WHAT IS IT? A service coordinator helps families find and schedule needed health, educational, early intervention, and social services.
WHO IS IT FOR? Children and young adults from the ages of birth to 21 years who are eligible for Medicaid who have a physical or mental disability that meets the medical necessity criteria.
WHAT WILL IT DO? Our Service Coordinators will provide the following:
Coordination/Advocacy
- Assist families in finding and scheduling needed services and
treatment
- Help families coordinate services
- Assist in utilizing natural supports in each community
- Advocate for the child and family for appropriate resources
Monitoring
- Ensure service plan is implemented
- Assess the child's progress toward outlined goals
- Ensure family's satisfaction with the services
Evaluation
- Determine appropriateness of the service plan, the need
for a new plan, or whether services are no longer needed
- Arrange needed evaluations
Crisis Assistance
- Coordinate activities needed in emergency situations
for the health and/or safety of the child
- Show the family how to access the service systems they need
- Empower and enable the client to function as highly
as possible in the least restrictive environment
